Peripartum Cardiomyopathy: Big Hearted Moms A Single Institution Case Series in Tangerang

Background: Peripartum Cardiomyopathy is a specific type of cardiomyopathy that presents with heart failure during peripartum period. This is a diagnosis of exclusion; thus it requires a high index of suspicion. Despite its rise in popularity lately, PPCM is still needs recognition especially in rural parts of Indonesia. There are still shortage of data regarding its prevalence and associated risk factors in Indonesia.

Method: This retrospective case series took place in a university hospital in Tangerang, where patients with Peripartum Cardiomyopathy were selected from the medical record by the ICD-10 code from the year 2013 – 2016. Gestational age, parity, length of stay, risk factors, clinical presentation, Electrocardiography, Echocardiography, and drugs used were obtained to further our understanding of the baseline characteristics and its association with the severity of the disease.

Result: From our study 18/26 (69%) were multigravida and 5/26 (19%) had pre-eclampsia. Mean age at presentation was 29 (18-40) with mean parity of 2.3 (1-5). Mean LVEF at presentation was 31% (15.3% - 47.8%), LV Thrombus as a complication occurs only in 1/26 (4%) without any thromboembolic events. Electrocardiography showed a sinus tachycardia rhythm in 24/26 (92%), and 5/26 (19%) had anemia. All patients received diuretic and ACE inhibitor/Angiotensin Receptor Blocker, while only 8/26 received anticoagulant (Warfarin) and 12/26 received beta-blocker. There was one mortality due to Eclampsia complicated by HELLP syndrome.

Conclusion: Our data suggests similar results of other single institution case series reported from elsewhere, in terms of onset, and probable risk factors associated with PPCM.
