Characteristics of Hypertensive Patients in Singkawang, West Borneo: Insight from a Multi-Cultural Remote Area in Indonesia

Background: The prevalence of hypertension are lacking in Indonesia. Furthermore, little is known about hypertension in a remote area like Singkawang city, West Borneo, which has multi-racial and cultural background. This study aimed to describe the clinical profile of hypertensive patients in Singkawang, West Borneo.

Methods: In this prospective study, we collected social demographic data including ethnic, abdominal circumference, risk factors, baseline electrocardiography, and total cholesterol from patients in South Singkawang I Primary Healthcare during September – December 2017. Inclusion criteria were systolic BP of >140 mmHg and/or diastolic BP >90 mmHg. Data were input and analyzed by using IBM SPSS 22.0.

Results: Out of 71 patients 35 (49.3%) patient were males, the mean age of 53.1±10.1 years. Majority 58 (81.7%) have stage II hypertension according to the newest ACC/AHA 2017 Hypertension Guideline, 31 (42.5%) were Melayunese, 56 (78.87%) were smokers, 24 (33.80%) has Diabetes, and 42 (59.2%) patients has central obesity as co-morbid. The ECG profile showed presence of LVH according to Sokolow-Lyon criteria in 34 (47.88%) patients and 6 (5.63%) patients had Atrial Fibrilation. Laboratory findings showed that 58 (75%) had hypercholesterolemia with mean total cholesterol of 255.77±66.83 mg/dl. Nineteen (26.7%) patients had hypertensive crisis at presentation.

Conclusions:

Most of our samples have stage II hypertension, with one fourth of them presents with hypertensive crisis. The clinical presentations of hypertensive patients in our samples are alarming. This is possibly due to the lack of education and other cultural beliefs, however further research are needed to confirm this.
