Background: Hypertension in an established risk factor for Peripheral Artery Disease (PAD). But the correlation between severity of hypertension with ankle brachial index is unknown. The aim of this study was to evaluate the correlation between level of blood pressure in hypertensive patients with ankle brachial index (ABI).

Methods: In this cross-sectional descriptive analytic study, patients who were suffering from hypertension defined with systolic blood pressure ≥ 140 mmHg and/or diastolic blood pressure ≥90 mmHg were included. These patients were between the age group 30-70 years. Patients with history of PAD, Diabetes mellitus (DM), Chronic Kidney Disease (CKD), and ABI value ≥1,4 were excluded from this study. Ankle brachial index (ABI) was measured in all subjects.

Results: The 52 hypertensive patients (mean age 57 years old, 38.5% male) were selected from internal medicine outpatient clinic NTB Province General Hospital. Prevalence rate of PAD (defined as ABI ≤ 0.9) and borderline PAD (defined as ABI 0.91 – 1.00) was 26.9% (14/52) and 53.8% (28/52) respectively. There was no significant difference of gender, age, and smoking habits between patients with and without PAD. Peripheral Artery Disease (PAD) was significantly more frequent in patients with hypertension stage II compared with hypertension stage I (43% and 16.2% respectively) (p = 0.033). Level of blood pressure in hypertensive patients had significant negative correlation with Ankle Brachial Index. The higher systolic blood pressure, the lower ABI (p = 0.012) and the higher diastolic blood pressure, the lower ABI (p = 0.034)

Conclusions: Level of blood pressure in hypertensive patients was correlate with prevalence of PAD diagnosed with Ankle Brachial Index.
