The Relationship of Congenital Heart Defects and Birthweight in Mataram City General Hospital, West Nusa Tenggara

Purpose: The most common birth defect is congenital heart defects (CHD) which has significant impact on morbidity, mortality, and healthcare costs. This paper reported the incidence and trends of CHD in Mataram City General Hospital.

Methods: This was a retrospective analytical study of the medical records of a 2-year period (January 2015-March 2017). All patients diagnosed by a cardiologist and considered suffered of CHD were included. Patients with more than one type of lesions were categorized into the defect that is hemodynamically most important. The patients were classified based on their characteristics. Data in each group were compared using the chi-square test or it’s alternative test (SPSS ver.23) and a p value of less than 0.05 was considered statistically significant.

Results: There were 91 CHD patients during the period of study. Number of CHD patients has increased from 20 in 2015 to 61 in 2016, and was predicted to be higher in the end of 2017. 85.8% of the patients were under 18 years old with ventricular septal defect (VSD) as the most common (47.3%). The incidence of CHD is 13.5/1.000 live births. Pre-term gestational age had a significant higher incidence (17.58%, p<0.0001). Very low (7.16%) and extremely low (35.71%) birth weight also had a significant higher incidence (p<0.0001). 35.2% of the patients were referred for surgical corrections. The mortality rate and fatality rate of CHD were 0.43 and 7.69, respectively.

Conclusion: There was an increasing trend of CHD patients in Mataram City General Hospital annually with VSD as the most common defect. The incidence of CHD is 13.5/1.000 live births. The incidence of CHD in Pre-term gestational age, very low birth weight, and extremely low birth weight were significantly higher than the normal group.
