Atrial Fibrillation is as a Predictor of in-Hospital Mortality of Acute Stroke Patients

Background: Atrial Fibrillation is the most common clinically relevant cardiac arrhythmia which increases the risk of ischemic stroke by 4 to 5 fold and is associated with increased morbidity and mortality. Recently, Stroke is still the leading cause of mortality in many countries. The aims of study is to investigate whether atrial fibrillation is a predictor of in-hospital mortality of acute stroke patients.

Methods: This retrospective cohort study of 192 patients were first onset of acute stroke (less than 24 hours) recorded in the Bethesda Hospital Yogyakarta Stroke Registry (2011-2017). These patients were divided into stroke patients with atrial fibrillation and stroke patients without atrial fibrillation. The primary outcome of the study was in-hospital mortality. The predictor of mortality were analyzed univariate and bivariate followed by the chi-square test. 

Results: Data of 192 patients with 96 stroke patients with atrial fibrillation, consist of fourty-three male (44.8%), and fifty-three female (55.2%). Fourty-five (46.9%) were more than seventy year old. The mortality of stroke patients with atrial fibrillation group is thirty-three patients (34.4%). Compare to stroke patients without atrial fibrillation consist of fourty-seven male (49.0 %), and fourty-nine female (51.0%). Twenty-four (25.0%) were more than seventy year old. The mortality of stroke patients without atrial fibrillation group is sixteen patients (16.7%). Bivariate analysis showed stroke patients with atrial fibrillation group is significantly associated with higher in-hospital mortality than in stroke patients without atrial fibrillation group (RR: 2.6, 95% CI: 1.3 – 5.1, p< 0.005).

Conclusion: Atrial Fibrillation is a predictor of in-hospital mortality of acute stroke patients
