Right Ventricle Function as Predictor Inhospital Mortality in Acute Coronary Syndrome Patientâ€™s in CCU Sarjito General Hospital

Background Acute Coronary Syndrome (ACS) refer to spectrum of clinical presentation ranging from ST Segment Elevation ACS to Non ST segment Elevation ACS (NSTEACS). ACS show that higher case inhospital mortality rather than non acute coronary syndorme. RV function is an important parameter in cardiac disease. One of quantitative approach to assess RV function is the measurement of the tricuspid annular plane systolic excursion (TAPSE). In several study showed that RV function can be mortality predictor in ACS patient
Aim: To describe TAPSE as predictor of inhospital mortality in acute coronary syndrome patient. Method : A Cohort prospective study was performed as a substudy of SCIENCE and ACS registry. We included all of patient admitted with acute coronary syndrome in Cardiac care unit sarjito hospital. Measure of RV Function with TAPSE, TAPSE < 17 mm indicates RV systolic dysfunction (reduce TAPSE).
Result : We studied 398 patients in January-october 2017 period. In baseline characteristic, there were no different parameters between both group, except age (63,08±11,51 vs 59,68±11,55, p=0,08), pneumonia (10,61% vs 2,8%, p=0,001), aritmia (7,96% vs 2,10%, p=0,001), ventilator (7,96% vs 2,10%, p= 0,006), creatinin (3,32±16,07 vs 1,37±1,03, p=0,041), ejection fraction (47,59 ± 13,13 vs 52,07 ± 11,50, p=0,001), we compare proportion of inhospital mortality in both group with the result normal TAPSE was 5,2% vs 20,35% in reduce TAPSE group. Relative Risk 3,9 with p=<0,001 CI 95% (2,09-7,14).
Conclusion : From this study, we found that RV Function can be predictor of inhospital mortality in patient with ACS.
